John was preceded in death by his parents James and Bessie Poulicek and his wife, Elizabeth "Betty" Poulicek.
He is survived by his loving daughters Laura (Bob) Lord and Lisa Poulicek; son John M. Poulicek; many nieces, nephews, cousins, family, and friends.
